Community-Rooted Provider Coalition

  • serves as the collective voice for physicians seeing low-income patients in Alameda and Contra Costa Counties
  • will ensure the perseverance and expansion of culturally competent points of access to health care
  • is designed as a model for preserving community practices, particularly those serving low-income people of color across the country

Criteria for membership

The Community-Rooted Provider Coalition is actively inviting qualified physicians, business associates and other interested stakeholders to become members. If you feel you meet the criteria outlined below, or if you have any questions, please contact us and we will respond promptly.

Full Membership

  • independent practicing physicians, clinics, dentists, psychologists, podiatrists, etc.
  • Must be a provider, medical director, owner or leader in an independent community practice.
  • Must have a strong interest in health equity for people of color, particularly residents of Oakland.
  • Must be committed to providing accessible, quality, culturally competent care to patients.

Dues: $400 per year or $125 per quarter for <3 years in practice; $600 per year or $200 per quarter for 3 or more years in practice.

Associate Membership

Business associates with an interest in health equity for people of color.

Dues: $600 per year or $200 per quarter.

Advertising space will be available on Community-Rooted Provider Coalition website and in print materials.

Affiliate Membership

Employed medical providers, other stakeholders and community-based organizations interested in health equity for people of color.

Dues: $125 per year or $40 per quarter.

Advertising space will be available on Community-Rooted Provider Coalition website and in print materials.

Resident and Student Membership

Must be enrolled in a medical school or residency training program and have an interest in health equity for people of color.
